St. Luke’s Foundation Awards $80,000 Grant
This grant enables Project Learn to continue its successful health literacy partnership with the Visiting Nurse Association (VNA). In addition, Project Learn will add to its practice a new partnership with Northeast Ohio Neighborhood Health Services (NEON). NEON is one of the oldest community-based clinics in the region and it serves some of Cleveland's poorest communities (which also have the highest rates of illiteracy).
Over the past eight years, Project Learn has worked with clinics, hospitals, health advocacy groups, home healthcare services and research organizations. It's helped providers institutionalize health literacy which ultimately establishes a permanent, long-lasting foundation for better communication between providers and patients.
